Sean, I am having the same issue. It appears to happen when using "Folders" view and mainly when moving or pasting a file. Also, the folder structure in the left pane of explorer changes, displaying different icons--it appears to be nonsensical and superficial, as though that instance of explorer simply has lost the ability to read the folders and files. (It does corrupt all instances of explorer which I might have open).
In addition, I also sometimes get an error "C:\folder\folder\file refers to a location that is unavailable..."
I don't have a solution (am searching for one!) except that I have found simply not using "Folders" view keeps the error from occuring, even after working for a very long time with multiple explorer windows open. Inconvenient though it may be.
