How do i set up a multiple field index?

In Table design view go to main menu...

View - Indexes

In the resulting dialog enter the name of your index in the first column
and one of the field names in the second column. To add more fields to the
index enter their field names in the second column on subsequent rows
beneath the first row and leave the first column blank.

Example of a three field index:

IndexName FieldName_1
FieldName_2
FieldName_3

If it is for a Primary Key in a table, Access will do it for you. Open the
table in design mode, select the fields by clicking the field selectors on
the left side of the window. Hold down the Ctrl key while selecting field 2
and above, then click the Key button on the toolbar.

To do this with other fields, open the table in design mode. Go to
View|Indexes. There will be 3 columns; Index Name, Field Name, and Sort
Order. Fill in the 3 columns for the first field. For subsequent fields in
the same index, leave the Index Name column blank. You need to enter fields
in the same index contiguously.
