Problems viewing my from in form view |

I have designed a form based on a query.I can view the form in design view but when I try to view the
form in form view all I get is a blank form .

When you open the query, do you see any records there? The form will go
completely blank if there are no records to display and no new records will
be added.
If that is not the issue, open the form in design view.
Select the Detail Section (grey bar at top).
Look in the Properties box (View menu).
Is the Visible property set to No?
