How to insert a record and redirect to another form(only if a selection is made from a list) |

request type
location
required equip ( 3 possible radio buttons )
Now if I complete everything on the form, and select radio button#1 or #2
the form should insert the record and tell them thank you.
NOW.... if I complete everything on the form and select radio button#3 I am
prompted to make a selection from a drop down list that contains 4 options.
I need the form to insert the record and re-direct me to the appropriate
form based on the selection I made.
Is this possible?
if they don't select radio #3 then it inserts and they are complete.
If they select radio #3 then they have to make a selection and they are
taken to another form( but of course the information they entered will
already be stored in the DB.
Is something like this documented anywhere?
Does anyone know how I can accomplish this?
